IMO Re:Zero is the only 10/10 isekai I've watched:

Yes, **** you, fight me but the mystery of Subaru's ability and WHY he is in the world is alone to pull you in. Most anime of the former are like "you dead congrats" or "chosen hero yadda yadda" but not here.
Only issue I have is Season 1 first half Subaru (but then he really does become cool by Arc 5) and how his allies are a bit too trusting of him too quickly without knowing his background (then again all isekai's do this and it annoys me.)

I'd also throw this in but then main character is a pervert to rival a whole red light district and show gets a bit too creepy at many times.

Konosuba, My Life as a Villainess, Saga of Tanya the Evil, Reincarnated as a Slime are all also good.

Isekai is a interesting concept that seems to get stranger the more I look at it.

Also I just realized the Wizard of Oz anime that I watched earlier tonight counts as a isekai. Since the original book was first published in 1900, that would make the isekai genre at least 122 years old.
