the most painful move is probably like an ftilt or some stupid ****, everything that has smash/smashlike/kill strength would cause you to explode on impact irl(been proven before) and in theory feel no pain due to the instantaneousness of the explosion.

So for the respawn thing i would say when you wake up and the sun is right in your eyes combined with someone pouring boiling water on your whole body. For the most painful i would say Bowser's fire or pikachu's lightning those 2 would hurt a lot.
