I said I couldn't think of one. Even the good "ugly" people/monsters/whatever just end up being cute.
Oh heck yeah attractive villains exist for sure. I can name quite a few myself.Sexy villains do exist though, but they are in moderation. Normally they would range from the brooding pretty boy anti-hero to the rugged calculating bad guy. Hell, even some fans may find that ugly or non-human villain attractive (Bloody Scar from the Lion King and Judge Frollo for example). Want an example of an attractive male villains in fiction, look no further than a majority of the Organization XIII.
Yes, even the "ugliest" member of the group is deemed attractive by the Kingdom Hearts fans.
The thing is though, Wario does fall under comic relief, sometimes he's the villian, and sometimes his uglyness is a gimmick.I actually have that one character that fits that role and their looks have never been an issue

I can think of one character who legit does avert all of the criteria, though.

-Huge ball of blubber, so I guess he's "ugly".
-Is a good guy. He's a loyal cop who just strives for justice, and people genuinely look up to him.
-While he has comic relief moments, everyone in Tekken does at times. It isn't a focus with Bob though, and he is genuinely looked up to. His quotes are generally fairly badass.
-His fatness was actually a choice - he built his body in a way that gives him as much aerodynamic, speedy, and flexible movement as any thin gymnast, but the weight from the fat and muscle allow him to hit like a truck.
-High to Top Tier character in every game he has appeared in.
It wasn't based off one game, it was pretty much based on every game Camelot has made since like 05 with World Tour being like the one exception, which I stated.

You were right, that night was pretty fantastic. I could remember it like it was yesterday and felt comfortable with the idea of watching a fanservice filled anime next to a beautiful girl like you (shocking for its to me). Your Pichu was a pretty mean mother and Kirby couldn't breath as long as he tackled him to death before the Thundershock KO. That's right, you did fit those qualities back then and you were someone who I can freely talk to about the things I'm normally afraid to talk about. That New Years event we had for example was pretty fantastic and I've never had that much fun with a guy in years (To Love-Ru marathon FTW). Though what was really funny was stomping your Kirby with Pichu around the same evening we had our Melee matches.
Those were memories that I would never forget.

Mileena looks soooo good in MKX, yet she's wearing more clothes and has smaller boobs than ever before. To me that only shows how sorry so many designers have become that they need all the goodies hanging out to make a character look good.

She does look great in MKX except for the mouth. It looks fine but it's really jarring with those lips considering her design over the years was looking like Baraka.
Yes, often less is more when it comes to attractive characters.
I make a small edit, but I did forget to say "bar MK9". Yeah, I hated the costumes in that game. The story is pretty topnotch, and at least Mileena doesn't bother me(she was always like that, but that seems to be something with Edenians and Shao Kahn's preferences anyway). Also, Shao Kahn made the Edenians wear even more clothing when he took over. Strange, isn't it?

I think the Klassic costumes look more attractive than their normal ones. (Seriously I love MK3 Mileena in HD and MK2 Jade as well)
This is why I don't mind her story costume. For one thing, she just got out of a test tube. She wouldn't be in full clothing at that point, just the most basic. Since being nude wouldn't be acceptable outside of an AO rated game, and even then... The other is it fits her personality perfectly. She's the only one in 9 I found worked with her story costume well. Well, besides the ones that weren't fanservice in a stupid way. Like respectively Shao Kahn who always looked like that, Liu Kang(same reason), and Stryker. There were some others that looked reasonable, at least, although I think all of them were male?
I would agree, if Mortal Kombat Deception didn't exist. I think that's when her sexyness/uglyness at the same time sort of deal really started kicking in. In fact, I always thought that MK9 Mileena was trying to be a lot like Mileena in deception.(Which isn't a bad thing at all, because before Deception, she was, like you said, just a boring psycho who wanted to be like her sister.)
