This isn’t a thread to ask for DLC or game-mechanics changes!
Patches Semi-Confirmation:
The Replay-Mode in Super Smash Brothers for 3DS tells us, that videos can get useless, if a new version of the game exist. This pretty much confirms the possibility of Gameplay-Patches!
After the release of Super Smash Brothers for 3DS in Japan the game already saw its first patch. This shows us, that Nintendo will support the game after its release. At least on the functional side. Mario Kart 8 also got a bigger patch recently, which even changed small inconveniences based on feedback from the community (like the cursor starting at “Next Race” instead of “View Replay”)
With Nintendo possibly collecting data for other patches, we also should start to gather bugs, glitches, inconveniences, balance issues and other problems, which bother every player and negatively influences the game. Besides informing other players about those issues, it is important to get vocal about those problems and giving analysis, journalists and others a source to go even more public with those information.
I will split issues in different categories. From game-breaking glitches to smaller inconveniences. If you want to add something to the list, use the following template. Please do not use insider-words of the community or abbreviations. Words, which are also used in-game (e.g. Power Shield) or are common videogame or game-design terms (e.g. Knockback), are completely fine:

Game-Breaking Glitches: ( 10 Glitches / Last Update: 02.10.2014)
If a problem breaks the game or the normal gameplay down, we have an Game-Breaking Glitch on our hand. In this category we count glitches, which have the player restarting his 3DS, the game or a match, as well as anything, which forces one player in a passive role and hinders the intended flow of the game.

Those are not game-breaking glitches, but they still can be encountered or setup easily on a normal match. They are obviously not meant to happen and can properly give a character an advantage or disadvantage. All in all they let the game look very buggy.

Yoshi will teleport after jumping from a ledge and landing with his Up-Special-Move on a ground.

Condition: Yoshi needs to hang on a ledge, jump form it on the stage and hit any kind of ground with the end of his Up-Special-Move animation.
Consequences: Yoshi will instantly teleport back to the direction of the ledge and also away from the stage in the air. Nonetheless the player can easily return to the stage again and use all his moves in the air.

Using a Counter Special-Move, when the ledge is taken, causes a strong backwards knockback on the user.

Condition: Ike, Marth or Lucina use their Counter-Moves, right after the ledge is taken from them. This can possible also affect Little Mac, Palutena, Greninja, Shulk and Mii Fighter.
Consequences: The character suffers from a strong backwards knockback, which may result in a KO.

Pac-Man's Fruits from his Neutral-Special gain momentum from Fire Hydrants (Pac-Man’s Down Special).

Condition: Running against water coming from a Fire Hydrant of Pac-Man’s Down-Special, while holding a projectile produced by Pac-Man's Neutral-Special
Consequences: The thrown fruit will gain momentum in the direction that the water was going towards. Either speeding up the projectile or making it go in the opposite direction.

The Player can affect Wario’s momentum after a knockback in weird ways.

Not every glitch has big consequences. Small problems, like missing sound or animation errors, can fall into this category. Also Game-Breaking Glitches, which need a very special setup and therefore are very hard to trigger under normal circumstances, can be seen as Minor Glitches.

Yoshi will jump out of reach, if he uses his Down-Special-Moves in the air followed up with his 1. Custom-Up-Special-Move.

Condition: The player has Yoshi’s 1. Custom-Up-Special-Move “High Jump” equipment. After the player uses Yoshi's one of his Down-Special-Moves in the air and Yoshi hits the ground, the player immediately uses his 1. Custom-Up-Special-Move “High Jump”.
Consequences: Yoshi will jump extremely high in the air, not lose a stock on the ceiling Blast-Zone and stay out of reach for every other player for circa 10 seconds. A player can use the glitch to wait out for an Time-Out or to annoy other players.

Consequences: Rosalina will not be damaged or get knockback by the moving road at the bottom of Mute City. She still will be influenced from the movement of the track, what can KO her.

Dominate Strategies / Advantaging Balance Issues: ( 2 Issues / Last Update: 02.10.2014)
Here I need to be special clear: I only will include issues, which have big consequence for the game and a character, and aren’t based on a glitch, but attributes or other standard elements of the game. If a player has no chance of escaping, something takes the fun of playing a character and overall hurts the normal flow of the game, i will be included in this list. In other words, if “Don’t get grab” is the only option for a player, while getting grabbed is a basic part of the game, then we can call it an Dominate Strategy or Balance Issue.

Diddy Kong can instantly cancel animations and hit-stun in the air with his Neutral-Special-Move.

Condition: While Diddy Kong suffers from hit-stun or goes through an animation in the air, the player inputs the command for his Neutral-Special-Move “Peanut Pop Gun” and cancels the move with the Shield-Button.
Consequences: While Diddy Kong should actually be suffer from hit-stun or go through the original animation, the player now can use Diddy’s Neutral-Special-Move or interrupt it with every another move. This allows him to interrupt any form of combo, multi-attack or lag from an animation. This gives him an unintended advantage in many different situations.

After Wario releases himself from a opponent grab, he can’t avoid a follow attack from different characters, including Little Mac’s KO-Punch.

Condition: The opponent uses one of following characters: Little Mac, Shulk, Zerosuit Samus, Rosalina & Luma, Shiek or Paulenta. The opponent grabs Wario and let him break free from the grab automatically.
Consequences: After Wario gets released from the grab, the player can’t act for a limited time. But this is enough time for many different characters to act before him, while Wario can’t avoid there attacks in any form. The Characters Little Mac, Shulk, Zerosuit Samus, Rosalina & Luma can even use some of their strongest attack against Wario and get a sure KO against the player.

Consequences: Although Mega Man should be suffer from the Hit-Stun of an multi-attack, he still can summon Rush, bounce upwards and thereby escape the opponent's attack. Therefore he only will suffer from the first few connecting hits of an mutli-hit attack. Yet he has no easy way to punish his opponent and take full advantage from his escape.

King Dedede’s Gordos can get stuck on his belly and disable his Side-Special-Move until the end of a match.

Condition: If King Dedede uses his Side-Special “Gordo Throw” , while he get hit with his won Gordo from an earlier use of his move, the new Gordo get stuck on his Belly.
Consequences: King Dedede’s Gordo will stay on his belly for the rest of the match and can’t be removed. If the player tries to use his Side-Special again, the Gordo will move in front, but stay there for moment and the return to the belly. The Gordo doesn’t damage anyone and will make the Side-Special useless. It gives the player with King Dedede a clear disadvantage, since he can't use his whole move-set.

Those are not game-breaking glitches, but they still can be encountered or setup easily on a normal match. They are obviously not meant to happen and can properly give a character an advantage or disadvantage. All in all they let the game look very buggy.

Yoshi will teleport after jumping from a ledge and landing with his Up-Special-Move on a ground.

Condition: Yoshi needs to hang on a ledge, jump form it on the stage and hit any kind of ground with the end of his Up-Special-Move animation.
Consequences: Yoshi will instantly teleport back to the direction of the ledge and also away from the stage in the air. Nonetheless the player can easily return to the stage again and use all his moves in the air.

Using a Counter Special-Move, when the ledge is taken, causes a strong backwards knockback on the user.

Condition: Ike, Marth or Lucina use their Counter-Moves, right after the ledge is taken from them. This can possible also affect Little Mac, Palutena, Greninja, Shulk and Mii Fighter.
Consequences: The character suffers from a strong backwards knockback, which may result in a KO.

Pac-Man's Fruits from his Neutral-Special gain momentum from Fire Hydrants (Pac-Man’s Down Special).

Condition: Running against water coming from a Fire Hydrant of Pac-Man’s Down-Special, while holding a projectile produced by Pac-Man's Neutral-Special
Consequences: The thrown fruit will gain momentum in the direction that the water was going towards. Either speeding up the projectile or making it go in the opposite direction.

The Player can affect Wario’s momentum after a knockback in weird ways.

Condition: After Wario got hit and blown away, the player holds upward.
Consequences: Wario can control himself in midair at amazing speeds and in weird trajectories. But he still suffers from the knockback, can accidently KO himself and use the move to counter an attack.

Minor Glitches: ( 7 Glitches / Last Update: 02.10.2014)
Not every glitch has big consequences. Small problems, like missing sound or animation errors, can fall into this category. Also Game-Breaking Glitches, which need a very special setup and therefore are very hard to trigger under normal circumstances, can be seen as Minor Glitches.

Yoshi will jump out of reach, if he uses his Down-Special-Moves in the air followed up with his 1. Custom-Up-Special-Move.

Condition: The player has Yoshi’s 1. Custom-Up-Special-Move “High Jump” equipment. After the player uses Yoshi's one of his Down-Special-Moves in the air and Yoshi hits the ground, the player immediately uses his 1. Custom-Up-Special-Move “High Jump”.
Consequences: Yoshi will jump extremely high in the air, not lose a stock on the ceiling Blast-Zone and stay out of reach for every other player for circa 10 seconds. A player can use the glitch to wait out for an Time-Out or to annoy other players.

Rosalina is not affected by Mute City’s moving road hazard in some cases

Condition: Rosalina is in one of the following states, right after using her Neutral-Special: Using her Neutral-Attack, Side-Tilt, Down-Smash, Shielding, Side-Special, Down-Special, Up- or Side-Taunt, slowly walking, changing walking direction, running, about to fall of an edge, during Landing Lag, using a Hammer, under the effect of a Super Mushroom.
Consequences: Rosalina will not be damaged or get knockback by the moving road at the bottom of Mute City. She still will be influenced from the movement of the track, what can KO her.

Dominate Strategies / Advantaging Balance Issues: ( 2 Issues / Last Update: 02.10.2014)
Here I need to be special clear: I only will include issues, which have big consequence for the game and a character, and aren’t based on a glitch, but attributes or other standard elements of the game. If a player has no chance of escaping, something takes the fun of playing a character and overall hurts the normal flow of the game, i will be included in this list. In other words, if “Don’t get grab” is the only option for a player, while getting grabbed is a basic part of the game, then we can call it an Dominate Strategy or Balance Issue.

Diddy Kong can instantly cancel animations and hit-stun in the air with his Neutral-Special-Move.

Condition: While Diddy Kong suffers from hit-stun or goes through an animation in the air, the player inputs the command for his Neutral-Special-Move “Peanut Pop Gun” and cancels the move with the Shield-Button.
Consequences: While Diddy Kong should actually be suffer from hit-stun or go through the original animation, the player now can use Diddy’s Neutral-Special-Move or interrupt it with every another move. This allows him to interrupt any form of combo, multi-attack or lag from an animation. This gives him an unintended advantage in many different situations.

Peach, Link, and Toon Link can cancel Hit-Stun / Aerial Lag by pressing Down-Special, while holding a Turnip / Bomb.

Condition: While holding a Turnip / Bomb in hand, do a short hop immediately followed by an airdodge. Upon Landing, press Down-Special and Peach / Link / Toon Link will instantly toss the item, bypassing the Aerial Lag caused by the Airdodge. This also works upon suffering any lag from an aerial, or when in Hitstun given by an attack.
Consequences: Peach, Link, Toon Link bypasses the ending lag or Hitstun given by aerials or attacks.

Not every balance issue has positive side-effects for a character. Some are very easy for other character to explode and take the fun away, playing those characters. The issue has to have his origin from the victim-side to be listed here.

After Wario releases himself from a opponent grab, he can’t avoid a follow attack from different characters, including Little Mac’s KO-Punch.

Condition: The opponent uses one of following characters: Little Mac, Shulk, Zerosuit Samus, Rosalina & Luma, Shiek or Paulenta. The opponent grabs Wario and let him break free from the grab automatically.
Consequences: After Wario gets released from the grab, the player can’t act for a limited time. But this is enough time for many different characters to act before him, while Wario can’t avoid there attacks in any form. The Characters Little Mac, Shulk, Zerosuit Samus, Rosalina & Luma can even use some of their strongest attack against Wario and get a sure KO against the player.

This category is kind of speculation and don’t have to be real issues. There about game elements, which don’t look like to be intended and aren’t glitches, but break the rules of the game. Since they don’t give a player a real advantage, i don't count them as real Balance Issues. Nonetheless they will be included for completeness.

Mega Man can escape multi-hit moves with his Up-Special-Move.

Condition: While Mega Man gets hit from a multi-hit move (e.g. Pikachu's Down-Smash) and uses the command for his Up-Special-Move "Rush Coil".
Consequences: Although Mega Man should be suffer from the Hit-Stun of an multi-attack, he still can summon Rush, bounce upwards and thereby escape the opponent's attack. Therefore he only will suffer from the first few connecting hits of an mutli-hit attack. Yet he has no easy way to punish his opponent and take full advantage from his escape.
