That list ONLY take MK legal tournaments into account. You can see Rajam's list on the tournament results forum. it's really, really cool and the community owes Rajam a lot.
The only issue with the list is that not everyone that is good goes to tournaments (ie me being above Shugo)

Why are people saying it's not real?
It's ELO, it's a statistical metric based off wins and losses.
It's probably more "real" than a standard PR ranking that tries to weigh biased version of who is good
The main criticism as Kel pointed out is that it rewards lots of attendance. Seem legit to me outside of that
