It's a male character that not just dresses feminine, but looks, talks, and acts with feminine mannerisms. You literally can't tell they're male (or female if it's the other way around) without being told.
Basically, the character designers drew a female character and hired a female VA and said "he's got a pair".
In anime, it's a bit different than a typical crossdresser and the two are generally used to different effect.
The term "trap" probably comes from the fact that these kind of characters are most commonly found in harem anime, which are about a guy being surrounded by a bunch of cute girls who fall for him, and he spends the series awkwardly dancing around which one he picks. When one of these extremely feminine male characters shows up and is making it clear they like the protagonist, the protagonist has effectively fallen into a trap as both answers to the situation end badly - trying to date the guy means going against the protagonist's sexuality and basically lying to the guy who likes him, but being honest also means hurting the poor guy who probably struggles emotionally as is.
More explicit shows might go a step further and only have the protagonist find out when he's in bed with the guy, which is obviously a whole other kettle of fish.

I tried to stay out of this stupid convo, because no one here is going to convince the opposing side to look at it their way. But everyone should drop it at this point. Because this is gonna get the thread into "YIKES" territory real quick.It's literally just a character design trope, though.
It's not really a real life concept. Actual real life crossdressers and actual trans people can't fall under the definition of a "trap" because you can still tell what their biological sex is, even if they're wearing clothes typical of the opposite sex. Unless you're like, really drunk or something, you couldn't accidentally end up with someone of the same sex who is trans or a crossdresser, or only realise their biological sex by them telling you. You simply can't get "trapped" in the awkward situation of having someone whom you thought was a girl confess to you, only to find out they are a guy. Even if they've gone under the knife, there are still telltale signs to a person's original sex unless they got an especially good surgeon to perform an especially thorough and drastic surgery, and said person diets and exercises to have the exact kind of body they want to reflect their gender, and wears the right kind of makeup. The amount of people who have the willpower and the money to do this is very low.
While it's entirely possible for, say, a male to be born and grow up to have some feminine physical features (I should know, I'm one of them), it's impossible for a male to look completely feminine without going through some tough procedures that I've already mentioned.
In fictional character design though, no such limitation exists, and that's all the term was ever meant to refer to. Anyone who calls anyone a trap IRL is using the term incorrectly.
