Bit of both. He knew the guy was there so he threw a grenade at him.

Except he didn't select a grenade when he threw it...

You can't select a grenade you have to set it before the match begins instead of a throwing knife, or maybe he thought it was a grenade anyway =/

Could have been he swapped and forgot he did? I know I've done that in TF2, switched to throw Jarate and found I had the machine gun.

all those doubles and triples is a mix of a good shot and a huge amount of luck, triple hs is just broke

I once shot a grenade out of the air with sniper rifle in halo 2, no scoped it too, it was just luck.
