i run and hit c-forward then c-up for uncharged sometimes, but mostly use a, then a + up quickly, since half the time i do a charged mortar. you really just have to practice focusing on doing those 2 moves as fast as possible. after a while you'll get it everytime. find the way that's right for you.

No, you don't. I play with the Wiimote/Nunchuk setup, and I just perform a manual usmash while in dash frames. No problems.

Don't do it at the same time. You have to hit up+z a split second after you c-stick down, and do it quickly.

If you're having trouble, practice in training mode at a slower speed, until you can do it at normal speed.

Dash forward, then rapidly rotate the thumbstick up while slamming the A button. It's really straightforward, there's no special technique neccessary and I discovered it accidentally.

It's a ton easier to do with the nunchuck for some reason...thumbstick sensitivity I think. Smashes are in general are easier with the nunchuck which is annoying because sometimes I do them accidentally.
