GIMP, Paint.NET, hmm, what else.. you could get Paint Tool SAI but that can cost money.

If you're looking for minimal editing, Paint.net will suffice. Otherwise, you'll probably have to deal with Gimp.

Can't think of something similar to SmoothDraw. I use Photoshop and SAI for my stuff so I wouldn't know. I heard GIMP crashes on 32-bit PCs, so if you're running a 64-bit PC, you'll be fine with it (that is, if you want to use GIMP), I guess.

Hmm. I think you'll be fine. I mean, I' no expert on why heads hurt, but it's no secret that I've experienced many different pains. Out of curiosity, have you hit your head? Stared at too many bright things? Jerked your head? It could just be that.

I have hit my head on a wall ones. got a Hematoma but that was last year. My sister thinks it is stress from college
