I'm not sure if this would be better suited to the brawl workshop help forum but this problem only seems to happen as a result of all three of the mentioned mods.
So I've done a lot of searching and only seen this problem mentioned twice on the internet and never explicitly solved.
I am working on a Project M build in which I have combined cBliss with 10 costumes each as well as the Custom Stage Expansion Pack with the 36 extra stages.
After replacing a single (any) icon for the SSS, all stage icons are changed to the Project M 'crossed out X' icon, and display the title of the stage you are currently hovering over inside all the stage icons. There were no issues with the build prior to this change.
I have attempted replacing the icons using brawl box, as well as stage manager; both have the same result.
The replaced stage titles, and large stage previews are working correctly.
I appreciate any assistance, I figure more people would have had this kind of problem before, as it was very easy for me to run into! I can provide any files you might need to help diagnose this.

Seems to have the same problem and claims to have fixed it by placing sc_selmap.pac in it's proper directory (which I'm assuming is either menu2 or system, regardless I have both sc_selmap and sc_selmap_en in both directories.) This method doesn't seem to solve my problem.
