"Disc cannot be read" error.

This error screen pops up and crashes my game whenever I see it. I don't have mods. It's more than an error. This error pops up whenever I pick certain stages, pick certain modes, or just randomly. My disc is clean, I can assure you. Please help. I don't know what to do.

It's probably a hardware problem, as the disc reader will eventually die if you use your system too frequently. But it usually takes a couple years for that scenario to happen.
