I'd say one of your major problems is positioning. It seems like you're having problems either not knowing where you want to be or not knowing how to get there. Because of this it feels like your main gameplan is to just kind of run away from the opponent where he can't get you and just hoping he screws up while chasing you, or seeing that they're not moving and hoping that when you charge at him your attack will hit and work out for the best. Watching your opponent closely can help you recognize where your safe positions are that let you hit them back easily.

Also, a couple of specifics:
Be careful with your recovery. A lot of times you were doing something that would either take you directly towards your opponent or leave you very vulnerable for a long time.

Watch how Peach is getting the down smashes on you. Typically you're getting down smashed because you aren't spacing properly (solved by watching her closer) or because she's just waiting for you to attack her while she's standing still and crouch cancelling a downsmash in response.


Your opponent really should be what you're focusing on at all points in the match because you should be able to tell what your character is doing and where your character is going to move and what's going to come out. If you have problems doing this then you need to make sure you can do whatever you want to do with your character whenever you want to do it and then just make a conscious effort to stare at your opponents character the entire time instead of yours.
