This has happened several times, usually with characters that spawn items.
I was playing as Sheik, and had picked up a Hitmonlee trophy after stage 2, and started playing around with her side-B special, trying to get her to move into the portal with it.
After the 3rd stage, I noticed I didn't get the trophy I'd picked up.
Same thing happened with Diddy Kong and his banana peel toss. Got rooked out of a couple trophies that way too.
Anyone know exactly what causes this? Not like I didn't have them already, just that it's annoying to lose something you're legitimately supposed to get.
