Would like some feedback on my Analysis

Just would like someoone's guide so that I'm on the right track with taking notes base on what has happen during a moment in melee. I'm doing this so that I can get better, by looking at high level players, how to avoid a punish, how to combo base on %, etc, etc. Some feedback would be great like what should i add or not add, and what's important.
