There are multiple ledge grab boxes that will grab the ledge when they intersect, unless the player is prevented from doing so by some other action. Some of these grab boxes will have the character go through a two frame transition period where they are vulnerable. When a character gets hit out of this transition period, the game will consider them as having grabbed the ledge already and restore the appropriate resources.
Actions that will prevent a character from grabbing the ledge include:
Being in an animation (specific animations enable certain ledge grab boxes such as recoveries).
Holding down on the controller.
Being hit recently (there’s a 54 frame lockout between getting hit and being able to grab the ledge)
Some animations will prevent you from grabbing ledge from behind, disabling that grab box.
