This happens because of knock back. I believe you can do this with other characters with similar type down airs like Sonic and Sheik. Easy to see/reproduce when you're at 60% and your opponent hits you with a up smash or something similar.

I've had the same problem. On Wii it's the build. Like Xermo said you are probably on WiFi or something is wrong with the build you have on your SD card. On Dolphin, your controller is 100% not set up correctly. Set it up like how Y-L says.

Some fine points by Shadowfury. Up air after down throw is much easier to connect than f-air and can juggle with more up airs if they do not air dodge or jump out. At higher % you can attempt down throw into up B. Try to dash attack less. You are not getting a lot of mileage from dash attacks...
