Read this guide: http://smashboards.com/threads/ledge-chain-grabbing-the-handoff-faq.216589/

The basic gist is...
1. Grab as Popo
2. Down-throw as Popo
3. Watch for the opponent to hit the floor. There will be a green flash that quickly appears. When this flash appears, input a grab for Nana to do.
4. Nana should get a grab.
5. If you are close to the ledge, Nana should Forward-throw
6. Move forward a little as Popo
7. Right as Nana says 'YA!', input another grab.
8. Get a grab as Popo.
9. Rinse and repeat.

Your grabs will have different timing on different characters due to the way Melee factors in the weight of an opponent to determine the speed of the throw.

The easiest character to do hand-offs on is Pikachu.

practice on pikachu. mewtwo also works, but sometimes his tail pushes you back and makes it weird.
once you're consistent on pika, practice on marth/sheik/peach and spacies and falcon/ganon.

also keep in mind, dthrow is effected by weight and stuff, fthrow is not.

nana also, on rare occasions, will get confused by the sloped edges on yoshi's. so sometimes she does really really dumb stuff, like bthrow randomly instead of fthrow.
it happens veeerry rarely, but just be aware of it.
