Happens on all set-ups that I've played on, easily reproducable.
Usually when someone is screen K.O.d they appear to fall through the stage rather than in front of it. I assumed that this was known and would be fixed after 3.5 but I never heard anyone talk about it. Maybe I'm the only one that notices it somehow but it's incredibly jarring and sloppy looking.
