Actully, they may have been mistakened, there is a white mario. So they might have mistaked that for Dr.M

Surposedly someone ripped the game disk and some characters, including mario have alternate costums of other characters. Mario surposedly has a Dr. Mario alt. How do you unlock? I don't know.

Seriously, exploring the disk folder contents has confirmed this. There is simply no folder for Dr. Mario in the fighter section.
